I/We, the parent's/guardian of the above named child, give my/our approval to participate in the Thrive Sports and Fitness programs. I/We assume all risks and hazards incidental to such participation. I/We do hereby waive, release, absolve, indemnify, and agree to hold harmless Thrive Sports and Fitness and their coaches, for any claim arising out of an injury to my/our child, whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance. I/We, hereby authorize Thrive Sports and Fitness and its assigns to utilize any and all photographs, pictures, or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in its promotional materials.
